Billing and Insurance Information
I am a private pay provider and am not in-network with insurance.
Payment is collected after each service rendered. Credit cards, HSA, and FSA cards are accepted.
I have a 48-hour business day cancellation policy. A shorter cancellation notice than this may result in a charge for the session. Please note that I may waive this fee on a case-by-case basis.
I do not bill insurance directly.

Superbill Documentation
To assist you with potential insurance reimbursement, I provide Superbill documentation for all services rendered. This documentation can be submitted to your insurance company for possible reimbursement.
Please note: It is your responsibility to check with your insurance company regarding their reimbursement policies for out-of-network providers. Reimbursement is not guaranteed and can vary based on your specific insurance plan.
CPT Codes
For your reference, here are the Common Procedural Technology (CPT) codes we may use for billing purposes:
Therapy Services: 90791, 90832, 90834, 90846, 90837
Evaluation Services: 90791, 98132, 96133, 96136, 96137
Telehealth Services: For any services delivered via telehealth, I add the -95 modifier to the above codes.
